El seguiente ejemplo es un formulario completo con la validacion de campos que son obligatorios
Para mostrar los valores en los campos de entrada después de que el usuario presiona el botón Enviar, agregamos un pequeño script PHP dentro del atributo de valor de la siguiente entrada campos: nombre, correo electrónico y sitio web. En el campo del área de texto del comentario, ponemos el script entre las etiquetas <textarea> y </textarea>. El pequeño script genera el valor de las variables $nombre, $correo electrónico, $sitio web y $comentario.
Luego, también debemos mostrar qué botón de radio se marcó. Para esto, nosotros debemos manipular el atributo marcado (no el atributo de valor para la radio botones):
